DEADLINE EXTENDED! Individual & Family Support Program FY2017 Funding _______

**Update 9/28/17:

The Department of Behavioral Health and Developmental Services has announced that, due to lower than anticipated applications received to date, the Individual Family and Support Funding Program will be extending the deadline for completing a draft Fiscal Year 2017 funding application to October 23, 2017.

Please note, the extension means that the first day for submitting an IFSP Application is no longer October 3rd. The new date is Monday, October 23, 2017 at 12 p.m.

All applicants should read the information posted on the web page for important updates on the new timeline, step-by-step instructions for submitting the application, and guidance for case managers submitting on behalf of families.

Applications will now be accepted from

12 p.m. on Oct. 23 through 5 p.m. on Nov. 17.

The new application submission date will not impact your receipt of the IFSP Funds, which is tentatively scheduled for early 2018. This time change allows us to make sure our outreach and training efforts are successful as we roll out this new system. Additionally, the IFSP staff will provide additional training to help those who need support with completing the application.

The Individual and Family Support Program has announced the Funding Period  for FY 2017.   

1)      The Maximum Amount of Request per individual remains at $1000

2)      Applications must be postmarked on or after November 15th, 2016

3)      Any applications postmarked before November 15th will not be accepted

  • Applications will be accepted on a first-come, first serve basis.
  • To be eligible for this program, an individual must be on  the new, combined IDD Waiver Wait List
